feat: 思考頭像即時記錄、失敗可重試，並加上 workspace lint
任務進行中可從思考頭像看到回合、工具與錯誤軌跡；失敗改成可分類的錯誤卡，
並可從 checkpoint 重試。同時補上 workspace clippy / cargo-deny 政策與開發文件。
